Step 1) Please print out and fill out the registration and payment form, (sign it and remit it to us through any of the following methods: Email the form to lascauxacademy@yahoo.com or lascauxacademy@gmail.com, or fax the form to (206) 337-1156, or mail to Lascaux Acadmey, PO Box 366, Belmont, CA 94002 If you have problem printing out the form, please call us at (650) 631-5888, or email us at lascauxacademy@yahoo.com or lascauxacademy@gmail.com to acquire the registration form.

Step 2) Remit your payment through any of the following methods: a) Check and money order: Send us a money order or a check (payable to Lascaux Academy) to the address below: Lascaux Academy  PO Box 366, Belmont, CA 94002 b) or online banking payment: add our school as a payee on your online banking (information you need): Payee’s name: Lascaux Academy Account number: Student(s)’ name(s) Payee address: PO Box 366, Belmont, CA 94002 Payee phone number: 650-631-5888 c) or credit card payment: please fill out the credit card authorization form below (we use Paypal to process credit cards): We accept major credit cards: VISA, Master, Discovery, American Express.
